Weekly Workout Planner

This Weekly Workout Worksheet can help adults keep track of their exercise routine for each day. The worksheet provides a space to get organized and also take notes for a 5-day workout week. Each day of the week can be assigned a different type of exercise–for example, Mondays can be strength exercises, Tuesdays can be for stretching, and so on. You can use the sample worksheet and fill in your specific exercises, or choose your own.

As always, consult your physical therapist and healthcare professionals to understand which exercises are safe or appropriate for you to perform.

After you’ve downloaded your worksheet:

  1. Fill each day's box with specific exercises that align with the given focus for the day (use the blank sheet to write in your own focus schedule).

  2. Every day, complete the day’s focus listed, and afterward, take notes on things like your energy level, what exercises you did, and anything notable about how you feel.

  3. Each week, once your worksheet is completed, you can review your progress and make other notes of any changes, whether they are positive or negative.

  4. Each month, you’ll have a routine to look at and adjust moving forward. It can also be helpful to track your progress for any particular goals for physical therapy.
